OnSight Optometry will (by way of email or direct mail) provide the following information to the Age Care Facility Manger/Clinical Nurse for each resident examined.
- Individual Ocular Report
- Summary Sheet of Ocular Health for all residents examined
- Summary list of Recommendations for all residents examined
- Resident Referral to be forwarded to the GP.
- A request for N.O.K details will be requested in order for the Optometrist to follow up any clinical recommendations.
Once assessments have been completed, reports are emailed or mailed promptly to the Aged Care facility.
